Treatment of congenital curvature of the penis

Peyronie's disease

In the case of congenital curvature of the penis, a corpoplication operation is performed, in which the upper, longer (convex) surface of the corpora cavernosa is sutured with non-absorbable sutures. At the same time, the penis is aligned, which normalizes sexual performance and psychological self-esteem.

In this case, the functional length of the penis does not decrease, since the distance from the pubis to the head of the penis remains the same. For men with initially small sizes, this operation can be supplemented with ligamentotomy, peno-scrotal angle plastic surgery, traction with an extender and a vacuum pump, injections of autologous mesenchymal stem cells, VEGF, which enlarges the penis by 2-4 cm. With reduced or subnormal testosterone, it is rational to carry out therapy to increase testosterone levels.

Often men also want to simultaneously increase the circumference of the penis, which can be done by installing a prolene mesh roller or collagen matrix impregnated with stem cells from a subcoronal (under the head) incision, which can add 2-4 cm to the circumference.

 

In the case of acquired curvature of the penis – Peyronie’s disease (specific fibrous compactions (plaques) form on the tunica albuginea of the penis, which lead to curvature of the penis. The penis is curved upward, to the side. Patients with signs of fibroplastic induration of the penis note pain during erection, which is not typical for congenital deformation), a corpoplication or corporoplasty operation is performed.

The fibrous plaque is removed and replaced with grafts from bovine pericardium, the patient’s own vein, or mesothelium grown from one’s own stem cells. The main advantages of the operation: removal of plaque, straightening of the penis, and the most important advantage – lengthening of the penis. In patients with deteriorated sexual function, it is rational to perform coporoplasty surgery with therapy and injections of mesenchymal stem cells for postoperative improvement of erection.
